Depending on the rate of absorption of the drug and factors that influence the absorption rate (e.g., taking some drugs after meals slows absorption), there will be peaking (high points) in the blood level and troughs (low points) shortly before or immediately after a dose is taken. Fabre (7), Domenjoz (8), Wilhelmi (9) and Yourish (10) have established the anti-rheumatic and anti-inflammatory activity of phenylbutazone. It is entirely unrelated to the steroid hormones. Toxicity of phenylbutazone has been investigated in rats and mice (11) and dogs (12). Phenylbutazone has been used by Camberos (13) in thoroughbred horses. Favorable results were reported in cases of traumatism, muscle rupture, strains and inflammations of the third phalanx. Results were not as favorable in the period treatment of osteoarthritis of the stifle and hip, arthrosis of the trapezious muscles and general arthritis. (7.5 mL), 1/2 fl. oz. (15 mL), and 1 fl. oz. (30 mL), each with an elongated tip for easy application and in dispensing bottles of 8 fl. oz. (240 mL). The preparation is intended for local therapy in a variety of cutaneous disorders of cats and dogs; it is especially useful in disorders caused, complicated or threatened by bacterial and/or candidal (monilial) infections. ACTIONS: By virtue of its four active ingredients, the ointment provides four therapeutic effects including anti-inflammatory, antipruritic, antifungal and antibacterial. Quadritop Ointment Indications: Quadritop Ointment is useful in the treatment of acute and chronic otitis of varied etiologies, in interdigital cysts in cats and dogs, and in anal gland infections in dogs. The preparation is also indicated in the management of dermatologic disorders characterized by inflammation and dry or exudative dermatitis, particularly those caused, complicated, or threatened by bacterial or candidal (Candida albicans) infections. Warning Clinical and experimental data have demonstrated that corticosteroids administered orally or by injection to animals may induce the first stage of parturition if used during the last trimester of pregnancy and may precipitate premature parturition followed by dystocia, fetal death, retained placenta and metritis. Corticosteroids administered to dogs, rabbits, and rodents during pregnancy have resulted in cleft palate in the offspring. In dogs, other congenital anomalies have resulted including deformed forelegs, phocomelia and anasarca. Precautions Quadritop Ointment is not intended for the treatment of deep abscesses or deep-seated infections such as inflammation of the lymphatic vessels. Parenteral antibiotic therapy is indicated in these infections. Quadritop Ointment (nystatin-neomycin sulfate-thiostrepton-triamcinolone acetonide ointment) has been well-tolerated. Cutaneous reactions attributable to its use have been rare. The occurrence of systemic reaction is rarely a problem with topical administration. There is evidence that corticosteroids can be absorbed after topical application and cause systemic effects. Therefore, an animal receiving Quadritop Ointment therapy should be observed closely for signs such as polydipsia, polyuria, and increased weight gain. Quadritop Ointment is not generally recommended for the treatment of deep or puncture wounds, or serious burns. Sensitivity to neomycin may occur. If redness, irritation or swelling persists or increases, discontinue use. Do not use if pus is present since the drug may allow the infection to spread. Keep out of the reach of children. Avoid ingestion. Oral or parenteral use of corticosteroids (depending upon the dose, the duration of use, and the specific steroid) may result in the inhibition of endogenous steroid production following drug withdrawal. Side Effects SAP and SGPT (ALT) enzyme elevations, polydipsia and polyuria, vomiting, and diarrhea (occasionally bloody) have been observed following the parenteral or systemic use of synthetic corticosteroids in dogs. Cushing's syndrome has been reported in association with prolonged or repeated steroid therapy in dogs. Temporary hearing loss has been reported in conjunction with treatment of otitis with products containing corticosteroids. However, regression usually occurs following withdrawal of the drug. If hearing dysfunction is noted during the course of treatment with Quadritop Ointment, discontinue its use. Caution Before instilling any medication into the ear, examine the external ear canal thoroughly so that the tympanic membrane is not ruptured, avoiding the possibility of transmitting an infection into the middle ear as well as damaging the cochlea or vestibular apparatus from prolonged contact. If hearing or vestibular dysfunction is noted during the course of treatment, discontinue the use of Quadritop Ointment. Dosage and Administration The frequency of administration depends upon the severity of the condition. For mild inflammations, the application may range from once a day to once a week. For severe conditions Quadritop Ointment may be applied two to three times a day, if necessary. The frequency of the treatment may be decreased as improvement occurs. Wear gloves during the administration of the ointment or wash hands immediately after the application. OTITIS: Clean the ear canal of impacted cerumen. Inspect the canal and remove any foreign bodies such as grass awns, ticks, etc. Instill three to five drops of Quadritop Ointment. Preliminary use of a local anesthetic such as Proparacaine Hydrochloride Ophthalmic Solution may be advisable. INFECTED ANAL GLANDS, CYSTIC AREAS, ETC: Drain the gland or cyst and then fill it with Quadritop Ointment. In studies of the mechanism of action, the minimum fungicidal concentration of clotrimazole caused leakage of intracellular phosphorus compounds into the ambient medium with concomitant breakdown of cellular nucleic acids and accelerated potassium efflux. These events began rapidly and extensively after addition of the drug. Clotrimazole is very poorly absorbed following dermal application. Gentamicin-Betamethasone-Clotrimazole: By virtue of its three active ingredients, gentamicin-betamethasone-clotrimazole ointment has antibacterial, anti-inflammatory, and antifungal activity. In component of efficacy studies, the compatibility and additive effect of each of the components were demonstrated. In clinical field trials, gentamicin-betamethasone-clotrimazole was effective in the treatment of otitis externa associated with bacteria and Malassezia pachydermatis . Gentamicin sulfate, USP Betamethasone valerate, USP and Clotrimazole, USP ointment reduced discomfort, redness, swelling, exudate, and odor, and exerted a strong antimicrobial effect. Tri-Otic Indications: Gentamicin-betamethasone-clotrimazole ointment is indicated for the treatment of canine acute and chronic otitis externa associated with yeast (Malassezia pachydermatis , formerly Pityrosporum canis) and/or bacteria susceptible to gentamicin. Contraindications: If hypersensitivity to any of the components occurs, treatment should be discontinued and appropriate therapy instituted. Concomitant use of drugs known to induce ototoxicity should be avoided. Do not use in dogs with known perforation of eardrums. Warnings: The use of gentamicin-betamethasone-clotrimazole ointment has been associated with deafness or partial hearing loss in a small number of sensitive dogs (eg. geriatric). The hearing deficit is usually temporary. If hearing or vestibular dysfunction is noted during the course of treatment, discontinue use of gentamicin-betamethasone-clotrimazole ointment immediately and flush the ear canal thoroughly with a non-ototoxic solution. Corticosteroids administered to dogs, rabbits, and rodents during pregnancy have resulted in cleft palate in offspring. Other congenital anomalies including deformed forelegs, phocomelia, and anasarca have been reported in offspring of dogs which received corticosteroids during pregnancy. Clinical and experimental data have demonstrated that corticosteroids administered orally or parenterally to animals may induce the first stage of parturition if used during the last trimester of pregnancy and may precipitate premature parturition followed by dystocia, fetal death, retained placenta and metritis. Precautions: Identification of infecting organisms should be made either by microscopic roll smear evaluation or by culture as appropriate. Antibiotic susceptibility of the pathogenic organism(s) should be determined prior to use of this preparation. If overgrowth of nonsusceptible bacteria, fungi or yeasts occur, or if hypersensitivity develops, treatment should be discontinued and appropriate therapy instituted. Administration of recommended doses of gentamicin-betamethasone-clotrimazole ointment beyond 7 days may result in delayed wound-healing. Avoid ingestion. Adverse systemic reactions have been observed following the oral ingestion of some topical corticosteroid preparations. Patients should be closely observed for the usual signs of adrenocorticoid overdosage which include sodium retention, potassium loss, fluid retention, weight gain, polydipsia and/or polyuria. Prolonged use or overdosage may produce adverse immunosuppressive effects. Use of corticosteroids, depending on dose, duration, and specific steroid, may result in endogenous steroid production inhibition following drug withdrawal. In patients presently receiving or recently withdrawn from corticosteroid treatments, therapy with a rapidly acting corticosteroid should be considered in especially stressful situations. Before instilling any medication into the ear, examine the external ear canal thoroughly to be certain the tympanic membrane is not ruptured in order to avoid the possibility of transmitting infection to the middle ear as well as damaging the cochlea or vestibular apparatus from prolonged contact. Side Effects: Gentamicin: While aminoglycosides are absorbed poorly from skin, intoxication may occur when aminoglycosides are applied topically for prolonged periods of time to large wounds, burns, or any denuded skin, particularly if there is a renal insufficiency. All aminoglycosides have the potential to produce reversible and irreversible vestibular, cochlear and renal toxicity. Betamethasone: Side effects such as SAP and SGPT enzyme elevations, weight loss, anorexia, polydipsia, and polyuria have occurred following the use of parenteral or systemic synthetic corticosteroids in dogs. These dosages produce sedation which is usually maintained for 1 to 2 hours and analgesia which lasts for 15 to 30 minutes. 2. Preanesthetic to Local Anesthesia - Xylazine HCl Injection (xylazine) at the recommended dosages can be used in conjunction with local anesthetics, such as procaine or lidocaine. 3. Preanesthetic to General Anesthesia - Xylazine HCl Injection (xylazine), at the recommended dosage rates, produces an additive effect to central nervous system depressants such as pentobarbital sodium, thiopental sodium and thiamylal sodium. Therefore, the dosage of such compounds should be reduced and administered to the desired effect. In general, 1/3 to 1/2 of the calculated dosage of the barbiturates will be needed to produce a surgical plane of anesthesia. Post-anesthetic or emergence excitement has not been observed in animals preanesthetized with Xylazine HCl Injection (xylazine). Xylazine HCl Injection (xylazine) has been used successfully as a preanesthetic agent for pentobarbital sodium, thiopental sodium, thiamylal sodium, nitrous oxide, ether, halothane and methoxyflurane anesthesia. Side Effects Emesis occurs occasionally in dogs, and frequently in cats, soon after the administration of Xylazine HCl Injection (xylazine), but before clinical sedation is evident. When observed, emesis usually occurs only a single time, after which there is no further emetic effect. The use of antiemetics may delay this phenomenon. The occurrence of emesis may be considered a desirable effect when Xylazine HCl Injection (xylazine) is administered as a preanesthetic to general anesthesia. Xylazine HCl Injection (xylazine) used at recommended dosage levels may occasionally cause slight muscle tremors, bradycardia with partial A-V heart block and a reduced respiratory rate. Should excessive respiratory depression occur following the use of Xylazine HCl Injection (xylazine), administer respiratory stimulants and provide artificial respiration. Movement in response to sharp auditory stimuli may be observed. Increased urination may occur in cats following the use of Xylazine HCl Injection (xylazine). Precautions Clinical results with xylazine have not revealed any detrimental effects when the compound is administered to pregnant dogs or cats. However, until more definitive studies are completed, Xylazine HCl Injection (xylazine) is not recommended for use in these animals. Careful consideration should be given before administering to dogs or cats with significantly depressed respiration, severe pathologic heart disease, advanced liver or kidney disease, severe endotoxic or traumatic shock and stress conditions such as extreme heat, cold, or fatigue. Analgesic effect is variable, and depth should be carefully assayed prior to surgical/clinical procedures. In spite of sedation, the practitioner and handlers should proceed with caution since defense reactions may not be diminished. Do not use Xylazine HCl Injection (xylazine) in conjunction with tranquilizers. Since an additive effect results from the use of Xylazine HCl Injection (xylazine) and the barbiturate compounds, it should be used with caution with these central nervous system depressants. Products known to produce respiratory depression or apnea, such as thiamylal sodium, should be given at a reduced dosage and, when injected intravenously, should be administered slowly. When intravenous administration is desired, avoid perivascular injection in order to achieve the desired effect. Studies have shown negligible evidence of tissue irritation, however, following perivascular injection of xylazine. Bradycardia and an arrhythmia in the form of incomplete atrioventricular block have been reported following xylazine administration. Although clinically the importance of this effect is questioned,1 a standard dose of atropine given prior to or following Xylazine HCl Injection (xylazine) will greatly decrease the incidence. While sedation usually lasts from 1 to 2 hours, recovery periods in excess of 4 to 5 hours have been reported in both dogs and cats. Because of these blood dyscrasias, test article was interrupted over four times more often in horses treated at the 2X dosage than those treated at 1X, although both groups were off treatment for about the same amount of time (approximately 20% of the treatment period). In some instances of bone marrow suppression, diet was supplemented with folinic acid. GI: Anorexia was observed in two horses in the 1X group and one horse in the 2X group. One horse in the 1X group and one horse in the 2X group were observed to be off feed. Observations of anorexia and decreased appetite occurred predominantly during the first 90 days of the treatment period. Observations of anorexia/decreased appetite in two of the above-referenced cases were due to unrelated illnesses. Loose stools were observed in three horses in the 1X group and five in the 2X group. The majority of these observations occurred in the first thirty days of treatment. Diarrhea was observed in one horse in the 2X group on Day 4 of the study. The appearance of loose stool/diarrhea observations was self-limiting and resolved without treatment or discontinuation of test article. Brief, mild colic was observed in three cases (one in the 1X group and two in the 2X group). Colic was treated conservatively or not at all and resolved without sequelae. Integument: Urticaria was observed in one horse in the 1X group and two horses in the 2X group. One horse was treated topically, two were untreated. All cases resolved without sequelae. Treatment crisis (marked worsening of the neurological condition) was reported in one horse in the 1X treatment group. Depression/lethargy was observed infrequently, occurred during the early part of the study in both groups and was primarily associated with the EPM syndrome. In one case, depression was associated with acute onset of a liver disorder. Seizure: One horse in the 1X treatment group suffered from seizures. Oral or parenteral use of corticosteroids (depending on dose, duration of use, and specific steroid) may result in inhibition of endogenous steroid production following drug withdrawal. Side effects: SAP and SGPT (ALT) enzyme elevations, polydipsia and polyuria, vomiting, and diarrhea (occasionally bloody) have been observed following parenteral or systemic use of synthetic corticosteroids in dogs. Cushing's syndrome has been reported in association with prolonged or repeated steroid therapy in dogs. Temporary hearing loss has been reported in conjunction with treatment of otitis with products containing corticosteroids. However, regression usually occurred following withdrawal of the drug. If hearing dysfunction is noted during the course of treatment with Animax Ointment, discontinue its use. Caution: Before instilling any medication into the ear, examine the external ear canal thoroughly to be certain the tympanic membrane is not ruptured in order to avoid the possibility of transmitting infection to the middle ear as well as damaging the cochlea or vestibular apparatus from prolonged contact. If hearing or vestibular dysfunction is noted during the course of treatment, discontinue the use of Animax Ointment. Warning: Clinical and experimental data have demonstrated that corticosteroidsadministered orally or by injection to animals may induce the first stage of parturition if used during the last trimester of pregnancy and may precipitate premature parturition followed by dystocia, fetal death, retained placenta, and metritis.
